Instructions for OnLine Submission

Please PRINT THIS PAGE and follow these instructions.

There are three ways to use the convenient, online application. Please read each option carefully to ensure your application is filled out and delivered properly.

  1. Capital Metro’s and/or Star Tran’s application may be downloaded by using the browser [File] [Save As] functions, saving the file to your hard drive with a new file name. The file can then be opened using Microsoft Word, filled in, saved and emailed as an attachment to: application@capmetro.org.
  2. The application may also be filled out directly from the browser, saved to your hard drive and sent as an email attachment to application@capmetro.org.
  3. The file can be filled out, printed, signed and faxed to 512.369.6010, or delivered via the U. S. Postal Service or other delivery service.
